Baxter & Walsh Physical Therapy, P.C. is conveniently located at 2237A Wantagh Avenue in Wantagh, NY 11793, between Sunrise Highway and Merrick Road. We are easily accessible by bus, car, or the Long Island Railroad, and provide spacious parking accommodations. Or at 1555 Sunrise Hwy, Suite 1 Bay Shore, NY 11706 (just west of the South Shore Mall)

Wantagh Office:

Coming from the East – Southern State Parkway West to Exit 28A South (Rte. 135 Seaford Oyster Bay Expressway). Take the Seaford Oyster Bay Expressway to Exit 2W (Rte. 27 Sunrise Highway). Go approximately 1 mile to Wantagh Avenue and make a left. Go approximately 1/2 of a mile and we are on the left side of Wantagh Avenue directly across the street from the Hess Gas Station and before Merrick Road.

Coming from the West – Southern State Parkway East to Exit 27 South (Wantagh Parkway towards Jones Beach). Take the Wantagh Parkway to Exit W6 E toward Merrick Road in Seaford. Turn right onto Merrick Road and get over into the left lane. The first intersection will be Wantagh Avenue. Make a left onto Wantagh Avenue and we are on the right after Dairy Barn and directly across the street from the Hess Station.

Coming from the East via Sunrise Highway – West on Sunrise Hwy. to Wantagh Avenue (approximately 1 mile after you cross over the Seaford Oyster Bay Expressway, Rte. 135). Make a left and go approximately 1/2 of a mile and we are on the left side of Wantagh Avenue directly across the street from the Hess Gas Station and before Merrick Road.

Coming from the West via Sunrise Highway – East on Sunrise Hwy. to Wantagh Avenue (first intersection after crossing over the Wantagh Pkwy.). Make a right and go approximately 1/2 of a mile and we are on the left side of Wantagh Avenue directly across the street from the Hess Gas Station and before Merrick Road.

Bay Shore Office (located in the MVP Building):

Coming from the West - Follow Sunrise Hwy. East to Exit 43 (Fifth Ave.). Stay on the service road PAST Fifth Ave. to the Exit ramp for Brook Avenue North. Brook Avenue will take you up over Sunrise Hwy., then make a left at the second traffic light on Mildred Place. Follow the road around to our entrance on the left.

Coming from the East - Exit Sunrise Hwy. at Brentwood Road (Exit 44). Stay on the service road past the mall and under the overpass. Make a right at the AUTO BARN STORE onto Esther Place and then a right into our parking lot.

Coming from the Southern State Parkway - Exit at Fifth Avenue (Exit 42 South). At the end of the exit ramp, make a left and then a quick right onto Fifth Avenue. Make a left at the CVS onto Brook Avenue. Follow Brook Avenue about a mile and make a right at the traffic light onto Mildred Place. Follow the road around and make a left into the parking lot.
